When the medical industry continued to fail his patients, he took into his own hand to save them but doing so outside the law. Introduced in Amazing Spider-Man #342 by David Michelinie, Erik Larsen and Randy Emberlin, Dr. Elias Wirtham decided to dedicate himself to become a doctor when his older brother died of an incurable disease. In medical school, he met Dr. Kevin Trench, and as his career began, he befriended biotechnician Gerald Stone and industrialist Tony Stark. When he discovered that there was a drug that could cure the same disease as his brother, he worked to buy up the company, where he learnt that the drug was actually ready long before his brother got the disease but was held back because it wasn’t profitable. Realising this drug could have save his brother, he became disgusted with Drug Industry and decided to do something about it.
Turning himself into a cyborg and taking the identity of Cardiac, one of his first target was Hammer’s Industries’ Sapirdyne Chemicals, who were manufactoring cocaine. He fought Hammer’s enforcers Rhino and Boomerang and had his first encounter with Spider-man. He tried to recruit the wedhead but Spider-man thought his method went too far. He would the target Stane’s International, where he encountered his old friend Gerald Stone, now Stone, who was parterning with Styx, a being of living cancer that was draining lifeforce from his victims. Looking for a cure for his partner, Stone accepted Cardiac’s help only for Cardiac to try and slay Styx. They fought and were stopped by Spidey.
After encountering Code:Blue and later helping Johnny Blaze and Vengeance with the child selling Child Inc., he would be reunited with Kevin Trench, now Nightwatch. Together they investigated Morelle. When Nightwatch was killed in The Great Game, he began hunting down the participant. He was later one of the many New York vigilantes that tried to stop a rampaging Wolverine but was defeated. He later set up a free clinic known as HEART, but would steal the medicine and tech to save his patients. This brought him to the attention of Superior Spider-man. When he learnt why Cardiac was doing ti for his patient, Otto agreed to help him. He would later be seen fighting the Goblin Underground and then recruited with other medics by Jane Foster’s Valkyrie to prevent Lady Death from dying as part of Marvel’s Medics.
